Almost all don't. Still can't figure out why it's such a rarity to find a flash in a camera phone.

optical zoom, real flash, sensor that is 10x as big, runs on 2 AA's, costs a fraction of a phone

probably a few reasons. first, they can only cannibalize other products so fast. flashes require a big flash and a big capacitor. definitely would impact size, which many makers may not want to tackle when they get $600 for phones without one. power usage would be huge comparatively. Finally i am not sure what kind of secondary circuitry would be required to deal with the power surges and such.
probably biggest reason is the cannibalizing vs. demand. people aren't demanding it yet.

The worst pocket digital camera beats the best quality phone-camera, any day. Try it and see. No one who seriously cares about quality photos is using a phone to take pictures.
This deal is at least warm, if I needed a cheap backup camera I'd order this without hesitation.

The iPhone 4/4S has it. The nexus s and galaxy nexus does as well. So does many other Samsung phones.
