Why is it that everything I buy lately has an extra 15% HST on top?

HST is 13%. Buying from the US is 0% if you stay for the required time. Plan your trips accordingly.

Why does this receipt have 13% HST and 5% HST at the same time?

No image, lol.
But you likely had items that fall under two brackets - 5% for those special 'exempt' items that got to keep the lower tax rate, 13% for everything else.
Not 18% on any single item. (or more if compounded)

The 5% and 13% HST rates occur in food places and there is a loophole in this. In Ontario, if you spend less than $4 (this should be raised) threshold then you pay 5% HST. If you spend more than $4, then you pay 13% HST.
The loophole here is if you can split all your orders into $4 (or less increments) then you pay 5% HST only instead of 13%. Take for example
Order A: Customer spends $8 at once and pays 13% HST for a total of $9.05
Order B: Customer spends $4 on first transaction and pays 5% HST. Then same customer orders another $4 and pays 5% HST. The total here will be $8.40
I do this loophole all the time at major food establishments.
And for those who voted Liberal in '07 and '11, you have yourselves to blame for this HST fiasco.

I assume that the OP sees a receipt that has the 13% HST printed on it along with a break down of the old GST / PST % as well. Add it all up and make sure you didn't get charged twice, but I assume it is just showing you what 5% of the tax is.
