If you deal with a travel agent, or book through the airlines' call centre, most international tickets have a stopover that can be added for a small additional fee (usually ~$100 + relevant airport taxes if necessary). This is cheaper than booking domestic segments/fares, and then adding on to an international fare.
The online booking websites traditionally don't do a very good job of implementing the entirety of the fare rules.
Japan is traditionally one of the more costly places in Asia to fly to (on a relative basis) because of the relative wealth of Japanese, and very high taxes in Japan on air travel.
