Looking for help configuring unlocked VTA-CV for TekSavvy

I've got an unlocked VTA-CV (D-Link's old Vonage box) and I've signed up for Teksavvy's TekTalk, but I don't know what settings to change and to what -- the only info that Teksavvy's given me is the username, password, and phone number.

Any help would be greatly appreciated!
