When you are approaching your data cap you are often send a text message warning.

I believe this is mandated by the CRTC to prevent high overage fees.
Does this apply to voice minutes/data as well?

Phone service provide should set a warning for the limit for the data and voice once you approach the limit

I know my rogers allows me to set notifications for any amount to alert me.

Data also gets frozen at $50 and $100 over and requires you to re activate it

You can set data threshold warning on any Android phone. Not sure about voice though.

Don't know and I don't think it should.

Data makes sense since it's (unreasonably) expensive and there are a lot of unintentional data leeches out there.

Voice and text? The customer should have an idea of their use through the month.

Time for the customer to accept some responsibility for their accounts.
