Here is the phone number to call from the Costco Garage Door special order brochure 1-866-496-2032. It says to call for a free in-home consultation. The call centre is owned and operated by an independent third party.

Good catch, deltone. The homepage of that site has been hacked to add some HTML code to load a script from another site that may infect your computer. I've disabled that link above.
