I'm pretty sure it's B.
Edit: I resently picked up a package and they converted the US amount to Canadian before calcualting the duty. No mention of shipping.

The last time I was charged duty, it was option d.

I got a UPS Express package a few months ago and it was B.
Shipping price wasn't part of the amount subject to the GST and PST charged to me.
