For some reason they submit your phone number and date of birth using two different formats. So just type out the same info twice.

If it was successful you should be taken to a page with a bunch of random info. Look for "({"success":true" If you see that at the top, you got your sample. It's very easy to send in multiple requests like this, so be careful (unless of course you want more than one lol).

Thanks, the facebook app is down, so I had to use the hard method. A little bit of playing around until I got it right. But it did show up as successful so hopefully there was still some left

This is their counter keeps track of how many samples have been redeemed(LOTS left btw if there is 30,000 total). Since the tab to redeem is down, it will be very easy to see if your order went through if you use the "hard mode" I added. After you get the "success" message, refresh the link above. If the counter went up, your order went through.

Just tested. Works great. Went from 13838 to 13839 right after I filled out the form.

If you are having trouble with the hard mode, the bold is what you need to change. It might be easy to just copy this into a text file and edit it with your own information(even though you will lose the bold font).
