Need website created with survey like assessment feature

Was wondering if anyone can recommend an individual or company that can create a website with a survey like feature that assesses after responses are entered and then sends a response via email.

Was wondering who was reputable and no it doesn't have to be in Canada.

You can probably get away with using Google forms, but you'll probably need a savvy younger type to figure it out.

I assume you want a website and not just survey? If you just want the survey, there are alot of companies that provide that like surveymonkey
