Promotion Advice
My wife was just offerred a promotion to management by her boss. Two days later she got her pregnancy test back as positive.
Should she tell her boss so that she could find someone else to run the new department and stay in her old position
or
accept the offer and not mention anything at all until 3 months (which I think is customary)
Not sure what the etiquette is.

Best thing to do is check her company's HR policies on this (and see if they indicate when notification should be given).
I think you don't need to jump the gun as you just found out and could wait a month to think it over and confirm everything.
If your wife has a good relationship with upper management and the company, you could probably tell them now if you wanted to strengthen the relationship.

Best thing is come clean. She should thank her boss for the show of confidence and say that she would really like to take the job, but blah blah blah.
If she waits it can be seen as being manipulative or worse.
Once the offer is made I don't think it can legally be taken back due to her condition. Most probably the promotion will move forward but contingencies will be made for her absence._______________

you can't be in trouble for not telling them, theres no need to tell them from day 1. Take to promotion jump in with both feet and then in a couple months tell them. It'll still give them 6 months before she takes her leave which is plenty of time.

As stated, take the promotion and then when it is necessary, tell them.
I don't want to jinx anything and god forbid anything happens but should she say "I'll stick to my current position" then miscarries? That'd be a double slap in the face. I am speaking from experience on this too. Do not make any concrete plans until at least the second trimester......_______________
