Reporting US Stocks Dividend Income? No T5

I have some US stocks that paid me about $40USD dividend last year.
I didn't get a T5 for them probably since they are less than $50, but afaik, we still need to report it.

I can actually go online (questrade) and look at the tax slip which shows the dividend I received.
However, although the slip says T5, it doesn't look like the standard T5 we get from banks (i.e. the standardized format and numbered boxes)

The tax slip basically say at the end: (3B) Gross Foreign Income (NON-US): $43.80

Should this amount be reported under box 15 "Foreign income earned as dividends"?
Also, should I be able to claim some amount on Box 16 "Foreign income tax paid on dividends income"? (The slip doesn't show this amount tho)

Did you go into your account and download the tax slips, the T5 should be there with all the info.

Gross foreign income is box 15
Foreign tax paid is box 16

that's the problem
there is no numbered boxes...it doesn't use the standard T5 format even tho the heading says T5.

The statement is from Questrade btw.
