I worked for a company for 5 years, I quit. I gave them 2 week notice. They new job wasn't a fit for me so I went back to my place of previous employment. This gap was 2 months. After 6 months I was laid off. Should I include this 2 month gap or leave it?

I would not mention that. it does not look good to a potential employer
