Yes, but remember that even if you turn the phone off and remove the sim, if a random text or wrong # hits TMO for your phone number you will be charged. Best at that point to set it to the $2 payg plan.

Has anyone with one of these SIMs temporarily transferred into, and then out of, a more permissive monthly plan? I will be in the US for about a month and would rather have the 5GB 4G data capability for the entire month as opposed to a 200MB cap a day.

Also, has anyone used these SIMs in a USB data stick, portable access point, or similar? I might take this approach for my month as well.

No, won't get charged. As long as you don't retrieve you voice mail (online)
As for txt. You can't retrieve if you don't put sim in and turn on the phone.
So no you don't get charged if you live it on pay by day. You just have to refill every 90 days to extend for another 90 days

My understanding (which has been confirmed by other users on this forum) is that you get charged when something (a phone call or text message directed at your phone) hits the TMo tower, even if the phone is off.

we confirmed here new-tmobile-pre-paid-plans-1530659/7/#post19600629
someone left voice mail to themselves and txt message, and didn't get charged
I can confirm also, I switch to pay per day on Aug. 16 and send txt message to myself and there was no charge

In terms of expiry for the balance, is there no more Gold rewards or anything else that can give extension beyond 90 days? Essentially, add $10 every 90 days is the only way?

either refill $10
or change to pay per minute and let them take $3 and switch back to Pay per day

Stupid question here

I have only been on $2/day plan and I have Gold Rewards until December, 2014 (I have never changed to another plan). If I put another $100, will I be grandfathered? In other words, will I be good for a year?

Silly question... Can you port a Tmobile number over to Roam mobility? They give you US numbers, so might as well keep mine. All this $3/ month 90 day refills is getting expensive.

If you flip into PayD, it's only expensive if you don't go to the states for 3 days (or more) for three months.

Not sure if someone has already posted but I use Lycamobile which is 3cents to canada and 2 cents of US. No need to reload as far as you can make one call every 90 days.
