You don't have to be 'old' to appreciate quality. 25 here and I would much rather pay a higher premium for quality fabrics and better construction that will last me for a long time over purchasing something that is really fashionable and cheap but will need replacing 1 or 2 months later, which is just a waste of my time.
That said, I own my fair share of club monaco and other mall brand products :P Sometimes it's just much easier to pick something up from a mall, drop it off to the tailor and pick it up within a week than waiting several weeks on MTM. But these are usually casual clothing.

Everyday work wear, my favorite is Canali. Fabric is great, nice tapered fit and the best spread collars (for the knots I use)of all the shirts I have... zegna collar is right up there as well, but the fit isn't as tapered.
My favorite slim fit dress shirts for business casual are from Paul Smith..nice tapered fit and feels nice. Not liking the collar though, so I'll only wear PS without a necktie.

I just tried on a h&m dress shirt and it fit very very well for off the rack, no alternations needed. They have a premium line at Eaton's and Yorkdale.
Although h&m is a hit or miss alot of the times._______________

All my dress shirts are mainly casual. I am 6'1" and thick in the middle (210lbs)so I have a hard time finding a shirt that fits. The h and m shorts might be cheap but they do the job. At less than $20 they are a nice fit for the price. Even though the material is thin, they don't fade. I've had a black shirt from there for a few years and after many many careless washes, it's still dark black. Other than that the white ones are way too see-through.
I have a few dressier shirts from club monaco that seem to fit very well too. A bit of a price jump ~$40 on sale.
Mexx shirts are not meant for bigger guys. I have never tried one that fits.

I have good success with Kenneth Cole. NOT Kenneth Cole Reaction which seems to be made for fat people with short arms. I was given something from Mexx and had to return it because it is apparent that you are not supposed to have a chest, biceps or forearms to fit.

are the Calvin Klein slim dress shirts form the Bay actually slim?...I know judging about other so call "slim dress shirts" from Mexx it was still bulky and I work out.

I'd like to buy my dress skirts from brand stores because they are designed in nice style with good quality. I would choose skirts with light color.

it's a shame Banana Republic doesn't make Slim fit XS size.
There smallest is small and the sleeves are just a bit too long for me.
The best fitting off the rack slim fit for me is Club Monaco. They fit better than the MTM ones i've gotten made.
