Lots of places can do this for you. A web search or yellow pages will find a whole bunch. Just be sure to get a few different quotes as prices on these kinds of things tend to vary dpending on who's giving the quote.

Some fixed costs will be the setup charge and the tape or stitching/embroidery charge. This is around $100 - $200 regardless of quantity. Make sure you ask for the tape so you don't have to pay again the next time you get something embroidered.
Check out Costco and Sam's club as well as premium/incentives companies like Idea Incentives , and of course my favourite resource....the government's own Strategis site

Here's the Costco program: http://www.brymark.com/costco/index.cfm
Fairly good pricing and not crazy quantities required._______________www.ehMac.ca - Canada's Mac, iPod, iPad & iPhone Community

The residential camp I volunteer at has used Highland Trophies and Sportswear for years. Excellent service, and good prices:

They're on Midland, near the 401, in Scarborough
Cheers!

This is a local company to me:
www.stitchgraphix.com
Good service, they do the embroidery right on site. I've used them numerous times for a variety of projects and always been pleased with the end quality. I know they'll do low quantity because we've had one-ups done there as well. While I've never shopped around on pricing, their bills always seemed reasonable to me.
(I know they also do high volume stuff as well).
