I bought my prescripted goggles at OMNI SWIM in Scarborough (Markham Rd/ Bellamy Rd). Basically, it's a pair of ordinary goggles, and you buy the lens separately. Not sure if they still sell those prescripted goggles there, but it's worth a try.

try national sports.
the location at sheppard/yonge used to sell them a few years ago. I don't recall seeing them in recent years but maybe give them a call.

Omni Swim is the place to go for swim stuff. Not sure if they sell them anymore, but I purchased a pair of prescription googles a couple of years ago. They were black googles that had lenses at different prescription levels, so you just got the ones closest to your prescription.
Nowadays I just wear contacts and purchase regular googles...much better, as long as you don't open your eyes underwater =)
