I'd never skip a midterm because of a cold.
In my entire undergrad degree I missed:
1 midterm because I was going on a ski trip. (prof let my write the night before, by myself)
1 final because I faceplanted and concussed myself on some ice. Went to see the prof, and as soon as he saw my face he told me I shouldn't write the exam - gave me the option of doing a makeup (which I did) or just averaging the rest of my marks.

Would you write a midterm if you had a bad cold, or write the makeup?
I really don't feel like sneezing and sniffling through a midterm tomorrow, but it's Sunday and I won't be able to get a dr's note until Monday. Make-up is on Friday, and by that time I should be all better. What would you do?
I'm too doped up on Ny-Quil to even cram tonight

If you feel that your sickness has impaired your ability to do well on the exam, write the makeup. The exception is if you really don't care about your marks and want to avoid the hassle and cost associated with a doctor's note, then write it on Monday.

I wrote some exams feeling pretty ******. But I didn't want the hassle of trying to get a note and writing the make up. Some profs I had said the make-up was generally harder, some also said they try to make it the same, but it ends up harder anyway because of the alternate questions they had. If you feel you will be useless in the exam dont write it.

The makeup exam will almost always be harder. Many professors know some students try to postpone them when there not actually sick which is why they will make it harder. Unless it is really bad then write the exam

Write it feeling 100%, there's no point in taking it when your sick. If you know your stuff it shouldn't matter which test you take. All I know is that the few exams I've taken while constantly sniffling/coughing feeling feverish have not been good times. And studying more is just going to stress your body out more and make you more sick. So just take the retake.

The make-up is usually harder than the actual exam... I'd write the midterm for sure unless my sickness impairs my ability to think on the day.

Being sick shouldn't make too much of a difference. If you're prepared, you'll do fine. I remember I wrote a vector calculus mid-term during a summer session with a fever of 102º. They had the central air blasting, and I was shivering half the exam and pouring sweat, while burning up the other half. I aced it.
Unless it's something that's really going to impair your ability to think and concentrate, then write it.

I missed it. I need more time to prepare anyways.

Make ups are usually harder, as everyone else has said. An MC-only test can become all written, which can be a disaster.

You're right in a sense, that this will go from an all-MC test to a test with diagrams and short answers, but I'm OK with that.
The MC for this course are what is known as multiple-multiples, which means there's only 35 MC questions on the exam, and each question has between 2-5 questions packed into it and if you only know part of the answer, you get a big fat zero for the question.
At least with short answer and diagram questions I can get partial marks.
