Well, there was no "hack" or backdoor access it was simply the improper use of a 3rd-party's legitimate login credentials.

However, "failure" can be looked at from the perspective that the authentication mechanisms are procedurally inadequate and lack the ability to ensure that login credentials are actually being used by the individual to whom they belong to (i.e. 2FA, etc).
