how to remove "Kodak Picture Kiosk II" reference from face book account??

few years ago i made the silly mistake of trying to send my photos from face book account to the photo kiosk at wallmart. my wife suggested to try the facebook login as its "easier" than a CD or bluetoothing them from my phone to the kiosk, and it was a nitemare i got locked out of my account and couldnt print my photos we had to come back with a data cd and get them done, fast forward to a year later, i see this thing on my face book activity called Kodak Picture Kiosk II how do i get rid of it?
