Resolved

The inquiry is resolved - there were a number of errors on the ROE hence the questions/confusion - this information wasn't available online but has now been clarified.

Most of your questions are self evident if you read the instructions for completing the ROE.

The start date depends on the specific facts...change of pay cycle, previous ROE issued, etc., so no blanket answer.
