For me...

Right click, open in private window, fill out the form selecting the first answer in each as mentioned above... Bingo!

Firefox on Mac, in case it makes a difference.

It was reported by several users and I still can't replicate your results with three different browsers using the methods above, that's why. You are free to move it back if you feel it's an edge case or I can un-expire it again.

I was able to complete the quiz by copy and pasting the link into a private tab but wasn’t able to download the coupon…but still +1, thanks anyway OP.

Just tested again, and it still works for me.

Mac/Firefox: Right click, open in new private window... Filled out the quiz, and seems like I'm getting another free sample.

I first tried Chrome incognito tab on Android phone, selecting different answers other than first option. Didn't get any sample. ☹

Then, I followed your method, Firefox private tab on Android phone, and it worked - sample!

I think incognito & picking the first answer for each question is key.
